The pages of this website do not collect, store, or process clinical records or health data. It is an information site. The exceptions are the booking form described above, and the patient feedback form, which asks about your experience of the service and not about your health.
Clinical consultations are provided by Dr Daniel Reardon, a CQC-registered independent doctor provider. Dr Daniel James Reardon is the data controller for clinical records. Clinical records, appointment data, and any health information shared during a consultation are held in Semble, a clinical records system that acts as a data processor on his behalf. For any enquiry about your clinical records, including a request for a copy, contact dan@drdanreardon.com.
We process contact enquiry data on the basis of legitimate interest, specifically to respond to your enquiry. We process patient feedback on the basis of legitimate interest: monitoring and improving the quality of the service, which is also expected of a regulated provider. Where you choose to leave contact details with your feedback, we process them on the basis of your consent. We process analytics data on the basis of your consent, provided by accepting cookies.
Contact enquiry data is retained for no longer than 12 months from the date of the enquiry, unless an ongoing relationship requires longer retention. Patient feedback answers are anonymous and are kept for as long as they are useful for quality monitoring. Any name and email left in the optional contact field on the feedback form are deleted 12 months after submission. Analytics data is aggregate, does not identify you, and is retained by Cloudflare under its standard settings.
